  • Unplanned escape of natural gas
  • Overpressure in a pipeline
  • Under pressure in a pipeline
  • No pressure in a pipeline that was known to be in service
  • Interruption of service
  • Natural gas fire, explosion, or accidental ignition on or near a gas facility
  • Unplanned flow-rate deviation
  • Pipeline damage
  • Activation of a safety device other than during a planned procedure
  • Unplanned status change
  • Interruption or failure of communications, control systems, or power
  • Gas pipeline damaged or broken
  • Unexplained outage to one or more customers
  • Inadequate odorization or reports of gas odor
